Health Care: Medical and Dental

Your medical and dental coverage continues for the end of the month plus one month in which the layoff or reduction in force occurs. You may continue your coverage through COBRA. You will receive forms for COBRA continuation at your address on file.

Flexible Spending Accounts

Health Care Flexible Spending Account
  • Before-tax contributions stop on the date of the termination (with your final paycheck). You may continue participation through COBRA. You may claim only those medical expenses incurred while participating. You have 180 days from the end of the benefit plan year to submit expenses incurred before the date of the layoff or reduction in force
Dependent Day Care Flexible Spending Account
  • Before-tax contributions stop on the date of the termination (with your final paycheck). You may not continue participation. You have 180 days from the end of the benefit plan year to file claims for expenses incurred before your termination date


 

Life, Accidental Death & Dismemberment, and Disability Insurance

All of your coverage in these plans terminates as a result of a layoff or reduction in force.

BenefitWhat Happens to Your Benefits
Basic Life
  • Coverage continues for 31 days after your termination date. During this time you may convert to an individual policy
Employee Optional Life
  • Coverage continues for 31 days after your termination date. During this time you may convert to an individual policy
Spouse Optional Life
  • Coverage continues for 31 days after your termination date. During this time you may convert to an individual policy
Child(ren) Optional Life
  • Coverage continues for 31 days after your termination date. During this time you may convert to an individual policy
Basic and Optional Accidental Death and Dismemberment
  • Coverage for both plans stops on your termination date. You may not convert or continue the Basic AD&D coverage; however, you may convert the Optional AD&D into an individual policy as long as you do so within 31 days
Basic and Optional Long-Term Disability
  • Coverage for both plans stops on your termination date. You may not convert or continue either plan
