Flexible Spending Accounts

- Before-tax contributions stop on the date of the termination (with your final paycheck). You may continue participation through COBRA. You may claim only those medical expenses incurred while participating. You have 180 days from the end of the benefit plan year to submit expenses incurred before the date of the layoff or reduction in force

| Dependent Day Care Flexible Spending Account |
- Before-tax contributions stop on the date of the termination (with your final paycheck). You may not continue participation. You have 180 days from the end of the benefit plan year to file claims for expenses incurred before your termination date

Life, Accidental Death & Dismemberment, and Disability Insurance
All of your coverage in these plans terminates as a result of a layoff or reduction in force.
