  • Coverage continues for the duration of your approved leave up to 24 months, provided you make any required contributions

Health Care Flexible Spending Account
  • If you are on an unpaid leave, you can continue to make contributions on an after-tax basis through COBRA. If you stop making contributions, you will be reimbursed for expenses incurred for only that period during which you made contributions.
  • If you are receiving a paycheck from the company during your leave, your contributions continue. Claims that you incur after your participation ends are not eligible for reimbursement. You have 180 days from the end of the benefit plan year to submit expenses incurred before your participation ends
Dependent Day Care Flexible Spending Account
  • If you are on an unpaid leave, your before-tax contributions stop and participation ends. You have 180 days from the end of the benefit plan year to submit expenses incurred before your participation ends

What Happens to Your Benefits

Basic Life

  • Coverage continues for the duration of your approved leave up to 24 months

Basic AD&D

  • Coverage continues for the duration of your approved leave up to 24 months

Basic Long-Term Disability

  • Coverage continues for the duration of your approved leave up to 24 months

Employee Optional Life

  • Coverage continues up to 24 months. For the first six months, you must continue to make your optional life contributions. After six months, and once you are approved for waiver of premium, your contributions are waived

Spouse Optional Life

  • Coverage continues for the duration of your approved leave up to 24 months, provided you make any required contributions

Child(ren) Optional Life

  • Coverage continues for the duration of your approved leave up to 24 months, provided you make any required contributions

Optional AD&D

  • Coverage continues for the duration of your approved leave up to 24 months, provided you make any required contributions

Optional Long-Term Disability

  • Coverage continues up to 24 months. For the first six months, you must continue to make your LTD contributions. After six months, and once you are approved for LTD benefits, your contributions are waived

Business Travel Accident Insurance

  • Coverage stops on the date your leave begins
